When Selective Cleaner is run in a well, and for reasons beyond our control, we are unable to complete the work, full charge for the job is made, according to the above price schedule. For the further protection of life and property, we carry public liability insurance as well as public liability and property damage insurance on our trucks. As the physical conditions in and around wells are not subject to our control, we can neither guarantee the results nor be liable for injuries to property or persons arising out of the performance of our field services. Furthermore, it is understood that the customer agrees to use all reasonable diligence and available facilities to recover instruments, tools and equipment lost in rendering our service and the customer agrees to reimburse us for the reasonable value of any such items or other personal property that cannot be recovered or repaired as well as for the repairs to recovered items. A representative of the customer must be present to direct elevations for all services. Information on services rendered is held in strict confidence. Whenever we are required to wait at the well because of well conditions or at the cus­tomer's request, a total of five hours' free waiting time will be allowed on each job. On waiting time in excess of five hours, a charge of $5.00 per hour will be made. Total waiting time will include the waiting time for the well to be made ready, stand-by time for reconditioning the well, stand-by time for testing, and the time required for extra setting-up or tearing-down of our equipment while operating. Should the customer desire to remeasure the well depth before proceeding with the job, and our depths are ultimately considered to be correct, the stand-by charges will be applied. If our depths are determined to be incorrect, the stand-by time will not be charged. When a truck is on waiting or stand-by time at one well and is given time out to serv­ice another well, waiting or stand-by time will not apply against the first well while the truck is away servicing the second well, nor will a set-up charge be applied against the first well after the truck returns to it. However, waiting or stand-by charges will be resumed when the truck returns to the first well. Except for Selective Well Cleaning and Electrolog Services, a set-up charge of $50.00 will be made when we set up our equipment and, through no fault of our own, are unable to render the service ordered and are released by the customer. On Gun Perforating jobs only, a set-up charge will not be made when we are unable to perforate due to well conditions if five or more perforating jobs in the same well have been performed during the preceding thirty days. After five set-up or perforating charges, or both, have been made for any one well within a thirty-day period, subse­quent set-up charges on that well will be waived when unable to perforate, providing such set-ups occur within consecutive thirty-day periods. A discount of 10% will be al­lowed on second or additional perforating jobs in the same well if performed within ten days after the previous job. Round-trip mileage will be charged when our equipment is ordered to the well and not used. The mileage charge is prorated when two or more jobs are performed on the same field trip. Under normal conditions the regular charges made for our services are those provided in the price schedule. However, in order to gain access to and from the well to be serv­iced, it is occasionally necessary to repair road beds and provide tractors, caterpillars, barges or other special means of transportation for our trucks. It is understood that the customer will make the necessary arrangements and pay the expenses involved. Terms: Gun Perforating, Bridging Plug, Merchandise Sales— Cash less 2 % 15th of month following date of invoice. All other Services, Labor, Mileage Charges— Net Cash 15th of month follow­ing date of invoice. Interest will be charged at the rate of 6 % per annum on all past due accounts. Invoices are due and payable at the point of origin. All prices are in accordance with O.P.A. regulations, and subject to change without notice. 11
