This is a card file and. consists of one or more cards for each account. These cards show location andhddress of account, breakdown of charges, existing rate, base rate changes and effective date of rates* 5) Form 4 "Water payment Receipts." This file of receipts dates back to July 1, 1951— prior to which time, no file is available. 6) Form 4-1 "Receipt For Water Service Connection Charge." This form shows service number, the amount charged, size of service connection, date of installation and location where installed. 7) Form 26 "Monthly Report of Water Servfeee Accounts." This form shows complete monthly financial status of each water service account. 8) Form 2? "Yearly Record of Water Service Collect!ons." This form shows date payment was received for each month of year. 9) Map record showing location of assigned service numbers. 10) Two card files— one of which is made up on the basis of customer names; the other on the basis of street addresses— -each of which shows the customer's service number. We have other files in the office relating to the operation of the Water Company in which I do not believe the District would be interested, but see no objection to making an agreement with the District which would provide for inspection of these records for a period not to exceed one year.
