Mr. J. K. Finlayson: In connection with your letter, January 27, file M 224-40 regarding the Group Insurance application of Mrs. Bessie B. Terry, she advises: *1 suppose they refer to the eye trouble I had, because that is the only ulcers I ever had. I believe it was caused by a little dust I got in ay eye as I was out in a dust storm shortly before this. I have had, no trouble since this one attack in September. I Just put some hot applications on my eye and the trouble went away.* As requested, I return herewith the copy of Form 5614, and will be glad to furnish any additional information desired.
