Nebraska (Name, Title and Address of Officer in charge of correspondence with the Commission regarding this report) GENERAL INSTRUCTIONS, DEFINITIONS, ETC. 1. This Form of Annual Report is prepared in conformity with the Classifications of Accounts prescribed for water utilities by the Commission above named in compliance with the statutes of the said State, and all accounting words and phrases herein are to be interpreted in accordance with the said Classifications. 2. The Respondent should make its annual report on this form in duplicate and file one copy, duly verified, in the office of the Commission on or before March 15, 195___, retaining the other copy in its files. 3. All entries should be made in permanent black ink, except those of a contrary or unusual character (like deficits, for example) which should be made in permanent red ink. When practicable, the report should be made by typewriter. 4. If it is necessary or desirable to insert additional statements for the purpose of further explanation of accounts or schedules, they should be legibly made on paper of durable quality and should conform with this form in size of page and width o f margin. This also applies to all special or unusual entries not provided for in this form. Where information called for herein is not given, state fully the reason for its omission. 5. Every Annual Report should in all particulars be complete in itself, and reference to returns of former years or to other reports should not be made to take the place of required entries except as otherwise specifically authorized. 6. This report form is designed to cover the calendar year. If the respondent makes a report for any other period, the beginning and the end of the period covered must be clearly stated on the upper half of this page and the appropriate cancellations of printed dates made. 7. The word respondent, wherever used in this report, means the person, firm, association, or company in whose behalf the report is made. 8. The word Commission means the Commission above named.
