These cards show location and address of account, breakdown of charges, existing rate, base rate changes and effective date of rates. 5) Form k “Water Payment Receipts." This file of receipts dates back to July 1, 1951--P*’ior to which time, no file is available. 6) Foim4~l "Receipt For Water Service Connection Charge." This form shows service number, the amount charged, size of service connection, date of installation and location where installed. 7) Form 26 "Monthly Report of Water Service Accounts." This form shows complete monthly financial status of each water service account. 8) Form 27 "Yearly Record of Water Service Collections."
