    Addendum To Cast Contract dated 4/14/97 PARKING Until further notice, entertainment employees may park in any regular parking space in the back parking lot. RECORDING TIME AND SIGNING-IN Employees are required to clock in for the first show between 6pm and 7:30pm and for the second show between 9:30pm and 10:15pm. Do not leave the backstage area after clocking in. Employees are also required to sign-in for each show after punching in. Failure to clock-in and sign-in may result in your not being paid properly. ON THE JOB INJURIES It is company policy that all injuries be reported immediately. You will be asked to fill out an accident report and turn it in to your supervisor before you leave the premises. If you are required to discontinue working because of your occupational injury, you must obtain a disability slip from your doctor. DOCTOR'S RELEASE If you file an accident report but do not see a Doctor and do not miss any work, you are not required to have a Doctor's release. If you have missed work or have seen a Doctor for your occupational injury, you must have a Doctor's release before returning to work. Ify^4russj/mdiia^ more days due to-eny illiiffSTorTTarr- EXTRA DAYS OFF If there are Extra people available to work, you may sign your name on the calendar for no more than two days off in a row. You may take more than two days off if no one else has requested the time and permission must be obtained through management. You may call to request a day off even if you were not signed up for it in advance. It is understood a written notice must be filed within 72 hours after a day off is granted. All extra days off are subject to checking after 4:00pm. You are not guaranteed the day off until you have spoken to management or received a message confirming the availability of an Extra person to work for you. Failure to check in will result in disciplinary action. Once you have signed up for an extra day off, if you must cancel, the cancellation must be made five days in advance. ADDITIONAL VACATION With management approval, you may request additional time off without pay. You may sign-up on the vacation list and the calendar for a vacation of no more than two weeks in a row, unless permission is granted by management for an extended period. Only one person at a time may take a vacation.
