( y> 1CERTIF1ED COPIES OF THE CERTIFICATE OF DEATH have been ordered from the Health Department for your use. The charge of $2.00 per copy, made by the Health Department, has been paid by Forest Lawn and added to your account. You should allow from one week to ten days for delivery of the certified copies. The Health Department will mail them directly to the address you have given us. Should the original Death Certificate be a "Pending" Certificate, issued by the Coroner, a longer period will elapse before you receive the certified copies. Please contact Forest Lawn if there is an unreasonable delay. If additional certified copies are needed, please contact the Health Department in the city where death occurred. The surviving spouse or, if there is none, the purchaser must go in person to the nearest Social Security Office to either file a claim or assign the benefits to Forest Lawn. If a surviving spouse is filing, the Social Security Card of the Deceased and the Certificate of Death, the marriage certificate and the birth certificates of all living minor children must be taken to the Social Security Office. The surviving spouse may file a claim for the lump sum benefit without having paid the funeral expenses. _______ If there is no surviving spouse, the person who pays the funeral expenses may file the claim upon presentation of- an itemized mortuary and cemetery statement showing the account paid, or may authorize payment of the benefit ^directly to Forest Lawn. ^___- ?╟÷ ?╟? ^___ __^* ?√φ FOR VETERANS BENEFITS BRING DISCHARGE PAPERS ?╟÷ Forest Lawn is filing the claim for the Burial Allow- ance. Your counselor will have you sign as the person authorizing the services, on Form VA 8-530. In this event, any payment made by the Veteran's Administration will be made directly to Forest Lawn, and your account will be credited with the amount of the payment.
